How can I mark diodes and LEDs on PCB to avoid confusion of Chinese PCB fab?

(On every board we submit a design for assembly, some designers may make PCB fab confused about the orientation of diodes or LEDs. And we had both Eagle's default diodes and KiCad's default diodes sent back from Chinese fab.)

I guess, we may add a silkscreen “+” and “-” just outside of the footprint on either end.  Just put the diode schematic symbol on the silkscreen. Very Clear.

Why did the SMT pad fall off easily when soldering the PCB boards?

I am a buyer in a telecom company. Recently, an important order of SMT PCBA is delayed due to insophiscated production of a supplier. I am thinking about replace it and just very puzzled about how did their SMT pad fall off easily when soldering the PCB boards. That slows down the whole process.

Do ICs have a shelf life?

What I would like to know is if ICs also have a limited shelf life and if so, do some types suffer more than others?
